I,__________________________________, agree to contact the Consumer Health
Services prior to fair to apply for a temporary health permit. I will present
the permit to the Big Horn County Fair Office before I enter the fair
grounds, and I will also pay the fee of $75 (min) - $300, depending on the
number of food items I sell. I agree to maintain healthful surroundings;
provide adequate cooling or refrigeration of food (where necessary); to keep
the area around me free from obstructions and litter; to remain open on the
above designated days and hours; furnish necessary building facilities; and
pay all permit fees and taxes.

The Big Horn County Fair Board will furnish the space. Electricity and water will be available.
